Home Health


  Medcenter One Home Health is designed to accommodate all types of healthcare needs in your home. Services for the treatment of medical conditions are usually prescribed by an individual's physician. You may require a single type of care or a combination of services. The goal of home health is to move you toward independence in the management of your health in your home.

Who may benefit from home healthcare?

Persons recently discharged from the hospital following illness or surgery
Newborns on home monitors or phototherapy
An elderly pateint who is forgetful and requires teaching about his/her illness and medications
An individual who is falling at home and may need physical/occupational therapy to evaluate safety and equipment needs or nursing visits to assess potential reasons for falls
Patients with illnesses that are high risk for readmission, frequent clinic visits or uncontrolled symptoms such as difficulty breathing or pain

What types of services does home healthcare provide?

Skilled nurses are RNs or LPNs with training and experience in providing care in the home. The nurse will communicate with your doctor to update your plan of care. We will partner with you to assess and coordinate needs and services. This may include skilled nursing care, help with medications, prevention of complications and education to help you reach your highest level of independence.
Physical therapists (PTs) work to restore mobility and strength of patients who are limited or disabled by physical injuries through the use of exercise, massage and other methods. They also teach patients and caregivers special techniques for walking and transfer.
Occupational therapists (OTs) help individuals who have physical, developmental, social or emotional problems that prevent them from performing the general activities of daily living (ADLs). OTs instruct patients on using specialized rehabilitation techniques and equipment to improve their function in tasks such as eating, bathing, dressing and basic household routines.
Speech language pathologists work to develop and restore the speech of individuals with communication disorders. Speech therapists also help retrain patients in breathing, swallowing and muscle control.
Dietitians provide counseling services to individuals who need professional dietary assessment and guidance to properly manage an illness or disability.
Social workers provide short-term counseling services, referral to and coordination with community resources and assistance with living arrangements, finances and long-range planning.
Certified nurse assistants (CNAs) assist patients with ADLs such as getting in and out of bed, walking, bathing, toileting and dressing. CNA services are delivered under the supervision of a registered nurse or licensed therapist.

How is home health paid for?
We accept payment for services from Medicare, Medicaid, Worker's Compensation, private insurance or private pay. For most services, there is no cost to you if you are an eligible benficiary for Medicare or Medicaid.

Call our office at 701.323.8400 and request to speak with a nursing supervisor.

How does 24-hour staff availability work?
If a concern or an emergency comes up at any time of day, at least one member of Medcenter One's home health team can be reached by a telephone number previously given to the recipient or the recipient's family. If the situation cannot be resolved over the phone, the staff member will visit the home. All on-call staff members are licensed and certified to work in a home health environment.
